Invoices now the right amount of needy

Just over a week ago, we allowed you to start making Invoices assigned to "No Practitioner". Upon release of that one, we heard plenty of feedback from people that use Cliniko by themselves saying that the Practitioner field now defaults to "none", which isn't too helpful for them!

We've now fixed this up, so that if you are the only Practitioner on your account, the Invoice will always default to your name!
