Appointment types are now grouped by category

We've just released a change to group appointment types by category in the appointment form. You'll find appointment types listed under their category's header instead of having the category added to the end of the name. Appointment types are in the same order as before.

If you don't use categories, there won't be any change to your appointment type list.
