A few tweaks for appointment type options in the appointment form

We've changed how the appointment type of a new appointment is selected for cases where a practitioner's default appointment type is not set or has been deleted. Instead of unavailable block, an appointment type that the practitioner does offer will be selected.

We've also made a change to how we list an appointment type that a practitioner does not offer. Previously, when changing the practitioner of an appointment to someone that does not offer the selected appointment type, the appointment type would change to Unassigned. Instead, we now change keep the same appointment type selected by adding that type as an option temporarily for the newly selected practitioner.
