Fix for editing invoices with inactive practitioners

We just released a bug fix that addresses an issue when editing an invoice associated to with an inactive practitioner.  

Before the fix, the practitioner dropdown didn't include the inactive practitioner when editing an invoice.  This meant that if you saved the invoice, it would be associated with a different active practitioner.

This is has now been fixed.
