Added "Average revenue per appointment" to new peformance report

We just added a new metric to the practitioner performance report. It is Average revenue per appointment.

This will help you to see who's either doing higher paid services, or who is selling more products with their services.

Most of you have finite room space, so increasing the value per appointment can be a good way to grow your business.

This has been added to the table at the top, as well as in the charts below.
