Recall updates and patient merge fix

Based on feedback we have made a slight change to the new recall feature with permission levels. Now, both practitioners and schedulers can create and update a recall as well as access the recall report.

Only administrators and power receptionists are able to create recall types, and delete recalls from patient records.

We have also fixed a bug where the latest appointment information was not updated when merging patients.

    Cool, any way we can get a pop up when the recall is due? On practitioner dashboard or something?
