No more confirmation emails for appointments in the past

It appears our initial estimates were wrong, far fewer patients are capable of time travel than expected. In light of this, it makes sense to cater the system to the more common "time challenged" patients.

We've made a change that when you create or change an appointment in the past, a confirmation email will no longer be sent out. This should be more desirable behaviour for the majority.

Sorry to those of you this affects negatively.
